Our meta-analysis included 60 randomized clinical trials evaluating the use of 8 antiarrhythmic agents for acute conversion of nonpostoperative atrial fibrillation 21, 102 ; . Because digoxin lacked efficacy compared with placebo 53, 55, 56 ; and -blockers and calcium-channel blockers were not definitely proven to be superior to placebo, studies that involved these agents or placebo were treated as having control arms. The studies evaluated heterogeneous patient samples, reflected by the conversion rates in the control groups, which ranged from 0% to 76%. Thus, the difference in absolute conversion rates among different antiarrhythmic agents is difficult to determine across studies. Figure 2 illustrates the absolute rates of conversion for the 43 studies with control arms. In pooled analyses, we found strong evidence of efficacy for acute conversion of atrial fibrillation compared with controls for ibutilide 103106 ; , flecainide 94, 107110 ; , dofetilide 111 116 ; , propafenone 94, 101, 117130 ; , and amiodarone 109, 112, 125, ; , with moderate evidence for quinidine 94, 123 ; Table 2 ; . Although efficacy was evident whether or not beta-blocking medication was given concurrently, rates of beta-blocker therapy withdrawal were not reported; hence, differential withdrawal of beta blockers from more patients in the placebo group may have exaggerated the apparent effect of amiodarone 840 ; . Pretreatment with either digoxin or verapamil does not reliably prevent postoperative AF 825, 841, 842 ; . Results with procainamide have been inconsistent, and this drug is not widely used for prevention of postoperative AF 843 ; . One report suggested that n-3 polyunsaturated fatty acids may be effective for prevention of AF in patients undergoing CABG surgery 844 ; . There is limited evidence that single-chamber and biatrial overdrive pacing prevents postoperative AF. In a randomized trial involving 132 patients undergoing CABG, postoperative biatrial pacing significantly reduced the incidence of AF in the biatrial pacing group by 12.5% compared with the other 3 groups 36% LA pacing, 33% RA pacing, and 42% without pacing; p 0.05 ; . The length of hospital stay was also significantly reduced in the biatrial pacing group 845 ; . A meta-analysis of 10 randomized trials comparing various types of atrial pacing to routine care after CABG surgery found that AF was reduced by RA pacing OR 0.68, 95% CI 0.39 to 1.19 ; , LA pacing OR 0.57, 95% CI 0.28 to 1.16 ; , and biatrial pacing OR 0.46, 95% CI 0.30 to 0.71 ; , but the number of enrolled patients was small and the pacing sites and protocols varied 846 ; . Discussion Am8odarone has several unique pharmacokinetic properties that remain to be fully defined. The acute single-dose absorption kinetics reveal a higher peak serum amiodarone concentration than that observed during long-term, lower-dose maintenance therapy. The disposition or disappearance of a single 800-mg dose of amiodarone is very rapid and there is no measurable amiodarone in the urine or feces after a single dose. Thus, the disappearance phase after a single dose of amiodarone probably represents predominant tissue uptake. Our postmortem studies corroborate these findings in that tissue concentrations of amiodarone were up to 300 times greater in most body tissues than in plasma. Fat and fat-laden organs contained the largest amounts of amiodarone, which suggests that amiodarone is lipophilic. Furthermore, after long-term administration, the terminal elimination half life is unusually long. The metabolism of amiodarone has not been fully unraveled. However, the metabolite desethylamiodarone has recently been identified by Holt et al.22 using an HPLC assay similar to ours. There is no evidence that this metabolite consistently accumulates in body.

This ancillary study of the Women's Health Initiative WHI ; randomized controlled trial found the contrary in women aged 65-79 years. Both women taking estrogen alone and women taking estrogen plus progestin had increased risk of significant decline in global cognition about 50% increased risk ; . In analyses pooling the effects of estrogen-alone and estrogen + progestin, hormone therapy increased the risk of probable dementia by 76% and the risk of probable dementia or mild cognitive impairment by 41%. Although not statistically significant, estrogen + progestin had a higher hazard ratio than estrogen-alone for the probable dementia outcome, 2.05 vs. 1.49. These negative findings are not included in the WHI global risk score, but should be included in discussions before and in continuing prescriptions of hormone therapy. 39 and captopril.

AFRAMODIAL Aframodial is an interesting molecule that has diverse medicinal properties. It has been shown to have anti-cancer, anti-malarial, and anti-fungal effects, although in the later stages of clinical testing it was found to lack an acceptable sideeffect benefit profile. For drugs that interact with protein targets, in either their efficacy, clearance, or toxicity pathways, model development is critical in the pursuit of redesign of active molecules with improved prospects. Information must be rapidly adduced to either suggest improved molecules within a class or abandonment of the given class owing to the generality of the limitations identified for a given compound. A recent very public example is represented by the issues identified with Vioxx, and how those issues related to the whole class of COX-2 inhibitors. ; The more information we can accumulate on the structure, properties, and function of these molecules and mode of action, the more rapidly improved drugs can be designed and made available, and the better our understanding will be of the molecular origin and generality of both desired and undesired effects. A classic example of this is provided by some very interesting research that has been performed on the underlying causes of the birth defects caused by women who used thalidomide during pregnancy and on the mechanism of gene damage [108, 109]. This is an example of how an understanding of the complete process is necessary to understand the adverse side effects and their molecular origin. Thalidomide, with improved understanding, is again emerging as a successful anti-cancer agent.
